• Intake Packet
• Copy of the front and back of your Insurance Card
• Copy of your Photo ID
• EAP Authorization number, # of sessions, & date span of authorization (if applicable)

You may submit your intake packet by:

• uploading to the patient portal
• emailing it through the patient portal
• emailing directly to me at drg@therapywithdrg.com
• faxing it to 844-398-7224
